He is an officer of the Harvard Business School Health Industry Alumni Association, the founder of Private Equity Analysts of Chicago, and a member of the Alumni Board of the Latin School of Chicago. |

| Aaron Lillybridge — Vice President Professional Development
Aaron Lillybridge is a Principal at Baird Capital Partners. He focuses on investment opportunities in the Manufactured Products sector with a particular focus on healthcare companies. Prior to joining Baird in 2005, Aaron spent two years with Galen Partners a private equity firm in New York. Before Galen, Aaron spent three years within the investment banking group at U.S. Bancorp Piper Jaffray completing numerous equity financings and merger and acquisition transactions. He received a BA in Business Administration from the University of Washington and an MBA from the Kellogg School of Management at Northwestern University with concentrations in Management Strategy and Finance. |

David Beecken is a founder of Beecken Petty O’Keefe & Company. Prior to founding Beecken Petty O'Keefe & Company in 1996, Dave was an investment banker specializing in the healthcare industry. Together with Ken O'Keefe, he was responsible for the healthcare investment banking practice at First Chicago (now J.P. Morgan) and a related joint venture from 1989 - 1996. In these two positions he directed, with Ken, twenty - five advisory assignments for healthcare companies aggregating more than $2 billion in transaction value. From 1979 to 1989 Dave was a managing director in the Chicago office of Smith Barney, Inc. Dave serves or has served on the board of directors of AbilityOne Corporation, DentalCare Partners, Inc., Heartland Information Services, the Patterson Companies (NASDAQ: PDCO), Reichert, Inc., Scrip Holdings Corporation, Sirona Dental Systems, TeraMedica, Inc., TIDI Products, and Valitás Health Services, Inc. | Jim Elrod — Managing Director — Vestar Capital Partners
Jim is a Vestar Managing Director, and is co-head of the Healthcare group. He joined the firm in 1998, bringing with him years of operational experience in the healthcare sector. He was Executive Vice President at Physicians Health Services, where he also served as corporate Chief Financial Officer, Chief Executive Officer of the New York and New Jersey subsidiaries, and President of the New Jersey insurance subsidiary. Before that, he was a partner at Dillon, Read & Co., a private investment bank, where he created and ran the firm's healthcare investment group, and served as co-head of equity issuance. Jim graduated with an AB, cum laude, in English from Colgate University, and holds an MBA from Harvard Business School. Originally from Atlanta, Georgia, Jim now lives in Riverside, Connecticut, with his wife and three children. |

| Jim Momtazee — Member — KKR
James (Jim) Momtazee has been a member of KKR Management LLC, the general partner of KKR & Co. L.P., since October 1, 2009. Before that, he was a member of the limited liability company which served as the general partner at Kohlberg Kravis Roberts & Co LLP (KKR). He works in the Menlo Park, California office and is a member of the firm’s Health Care industry team. In addition to the HCA merger agreement, he has been directly involved with the investments in Accellent, Alliance Imaging and Jazz Pharmaceuticals. Jim also serves on the boards of Accellent, Inc. and Jazz Pharmaceuticals, Inc. Prior to joining KKR, Jim was with Donaldson, Lufkin & Jenrette, where he worked on a broad range of corporate finance, mergers and acquisitions and merchant banking transactions. He holds an A.B. and an M.B.A. from Stanford University. Jim has lived in the San Francisco area for the past 16 or so years, but was raised in St. Louis and continues to follow its sports teams. |

Mr. Rhodes is a Principal in the San Francisco office, and is engaged in the firm’s investment activities in the healthcare services and pharmaceutical / medical device sectors. He is involved with TPG’s investments in Biomet, Surgical Care Affiliates (carved out from HealthSouth Corporation) and TPG’s recently announced investment in IMS. Prior to joining TPG's San Francisco office in 2005, Mr. Rhodes worked at McKinsey & Company and Article27 LTD, a start-up software company. Prior to co-founding Madison Dearborn Partners, Mr. Sullivan was with First Chicago Venture Capital for three years after having served in the U.S. Navy. Mr. Sullivan concentrates on investments in the healthcare sector and currently also serves on the Boards of Directors of Sirona Dental Systems, Inc. and VWR International, LLC. Robert Weltman — Managing Director — GenStar Capital Robert J. Weltman is a Managing Director of Genstar Capital, responsible for sourcing and executing acquisitions and monitoring several of Genstar Capital’s portfolio companies. Rob worked in the corporate finance and mergers and acquisitions departments of Salomon Brothers Inc. and Robertson, Stephens & Co. where he assisted technology, environmental services, real estate, basic manufacturing and service companies in initial public offerings, private placements and public debt offerings. Karen Kajmo is the Executive Director of HCPEA and is responsible for its day-to-day operations. Prior to HCPEA she worked as a software Product Manager at both enterprise and venture funded companies. At IBM Ms. Kajmo was responsible for managing the International Notes Corporate Consortium, a strategic customer group comprising (at that time) 20% of software revenues. In addition to her years in the software industry, Ms. Kajmo was a management consultant at PricewaterhouseCoopers assisting Fortune 500 companies on information technology strategy. |

Brian Miller is a Partner and Co-Founder of Linden Capital Partners, a Chicago-based healthcare and life science focused private equity fund. Prior to Linden, Brian was a founding member of the healthcare and life science team at First Chicago Equity Capital. Mr. Miller began his career in the investment banking division of Salomon Brothers Inc. (currently Citigroup). He is currently a board member of BarrierSafe Solutions International, CORPAK MedSystems, and Hycor BioMedical, and was a board member of Suture Express.
Andrew is a Founder and Managing Director of DW Healthcare Partners, a private equity fund that invests exclusively in healthcare companies. He has and currently sits on the Board of Directors of Genesis Technology Partners, Verathon, Tandem Labs, Reliant Renal Care, Pentec Health, Career Step and St. Andrews Management Services. Prior to co-founding DWHP, Andrew was Vice President of Business Development at Ventro Corporation, a builder and aggregator of electronic marketplaces. Andrew joined Ventro prior to its IPO and was involved in the strategic planning and capital raising process, in which Ventro raised over $40 million in private capital and over $360 million in the public markets, both in the U.S. and Europe.
David is a Principal responsible for acquisitions, divestitures and oversight of portfolio companies. David has been working with the Cressey & Company team since 1999 when he joined Thoma Cressey Equity Partners as an Associate. David rejoined the firm in 2004 after receiving his MBA. David also has worked with the healthcare investment banking group of Bear, Stearns & Co. Inc. and restructuring advisor Miller Buckfire. David earned an MBA, with honors, from the Wharton School at the University of Pennsylvania and a BS, summa cum laude, from Indiana University.
Darren Black joined SVLS in 2003, became a Partner in 2008 and is focused on healthcare service, pharmaceutical service and healthcare technology investments. Darren currently serves on the boards of Ricerca, Vitalize Consulting Solutions, CRI Worldwide, CR, Sequence Pharma Services, ISG Holdings and Spectrum Professional Services in addition to several active board observer roles (including Broadlane). Before joining SVLS, Darren was Co-Founder and President of two companies: ClinCare, a site management organization and PharmaStar (sold to United BioSource Corporation), a central nervous system education services company. Earlier he was a healthcare consultant for Accenture, assisting managed care and provider-based clients on strategy formulation and implementation.
Jeff is a Managing Director of Bain Capital Ventures. Mr. Crisan joined Bain Capital Ventures in 2002 after working in Bain Capital's Private Equity group since 1998. Mr. Crisan focuses on healthcare, education, and technology investments, including healthcare services, healthcare IT, for-profit education, software, and technology-enabled business services. While at Bain Capital, Mr. Crisan focused primarily on technology and healthcare investments. Prior to Bain Capital, Mr. Crisan was a consultant with Bain & Company. Mr. Crisan received an MBA with Distinction from Harvard Business School and a BA, magna cum laude and Phi Beta Kappa, in Mathematics and Government from Dartmouth College.
Mr. Koo is a Senior Partner of RoundTable where he is involved in all aspects of the transaction process, including deal sourcing, transaction structuring, valuation, due diligence, negotiations, financing and execution. Mr. Koo joined RoundTable at its inception as a Principal. Mr. Koo has significant investment banking experience and served as Vice President of U.S. Health Care Investment Banking, CSFB from 1999 – 2001.
David Neighbours is a Partner of Waud Capital Partners. Prior to joining WCP, Mr. Neighbours worked for Citigroup Investments in New York where he was responsible for the analysis, due diligence and execution of numerous private equity, mezzanine and structured product investments in a variety of industries, including communications, consumer products, financial services, healthcare and leisure. Previously, Mr. Neighbours was with Salomon Smith Barney, where he worked in the firm's proprietary investment group, SSB Capital Partners, and prior to that, in the firm's investment banking division.
onathan Oka is a Vice President of Flexpoint Ford. Prior to joining Flexpoint Ford, Mr. Oka served as an associate of GTCR. Mr. Oka has also worked at McKesson Corporation and as a mergers and acquisitions analyst with Lazard Freres and Co. in New York.
David Tamburri is a Partner at Health Enterprise Partners. Prior to joining HEP, Mr. Tamburri was a Vice President with Susquehanna Growth Equity, a private equity group focused on growth capital opportunities in the technology sector. He was formerly the President and Chief Operating Officer at Onward Healthcare, Inc., a provider of human capital services to healthcare providers sponsored by Welsh, Carson, Anderson, & Stowe. As an Executive Vice President with Pinnacor, Inc., a provider of information and analytical applications, David managed the company’s worldwide operations. Pinnacor completed an IPO in 2000 and was later sold to CBS Marketwatch.
